The Challenge

Safety teams at a large industrial facility were conducting hazard assessments and near-miss reporting using paper forms, which were then typed up into spreadsheets at the end of each shift. This created a lag between a hazard being identified and it being escalated for remediation — sometimes 24 hours or more. Inspectors couldn't attach photos or reference the facility's asset register from the field, and management had no dashboard showing open hazards by zone or risk severity. Regulatory audits required pulling records from multiple sources, a process that took days.

Our Solution

We built a mobile-first hazard reporting and safety compliance app for Android tablets used in the field, backed by a web-based operations dashboard. Inspectors can create structured hazard reports against specific zones and equipment assets, capture geotagged photos, assign risk severity ratings, and designate responsible teams — all from the field, with the data syncing to the central system within seconds. The dashboard shows a live heatmap of open hazards by facility zone, tracks remediation progress, and sends escalation alerts when high-severity hazards aren't actioned within a defined SLA. Audit-ready compliance reports can be generated in one click.

Our Approach

Safety Workflow Mapping

Conducted on-site observation sessions with safety officers and EHS managers to document the full hazard identification, reporting, and remediation workflow. Identified the data fields, classification hierarchies, and escalation rules that needed to be codified in the system. Reviewed existing paper forms and translated them into a structured digital schema.

Mobile Reporting App

Built an Android tablet app with offline capability using SQLite local storage. Inspectors can complete assessments, attach photos, and record observations with no connectivity, with all data syncing to the backend when on the facility WiFi network. The form structure enforces required fields and risk classification before submission.

Escalation & Notification Engine

Built a rules-based escalation engine that assigns remediation ownership based on hazard category and location zone, then sends push notifications and email alerts. High-severity hazards trigger immediate notifications to department heads and the EHS manager. SLA breach warnings are sent if a hazard remains open past a configurable threshold.

Dashboard & Compliance Reporting

Delivered a web dashboard showing open hazards plotted on a facility zone map, broken down by severity and department. Built one-click compliance reports covering open and closed hazard counts, average resolution times, and recurring hazard locations in a format aligned with regulatory reporting requirements.
